MAMI Assessment Form (with Feeding and Maternal Mental Health Assessment)

After initial rapid screening in the community, any mother-infant pair with an identified risk is referred for a full assessment at the nearest health facility. The MAMI Assessment form can be used for a more thorough assessment to identify specific issues to inform the type and level of care needed. This form screens for danger signs, clinical symptoms, growth of the infant, feeding risk of the infant.
In addition to the infant, the mother is assessed for key maternal health, nutrition and maternal mental health concerns. If there is any sign of concern in any initial screening the MAMI Maternal Mental Health Assessment Form can be used to determine level of risk (low to high).
The assessment helps provide the case worker with an in-depth classification of risk. This can take place wherever there are suitably qualified/trained staff; this is likely to be at an outpatient clinic or hospital in most settings.
